At Lead the Way, we have a highly skilled team with diverse skillsets to work alongside NDIS participants to achieve their individual goals. We provide our clients a trauma informed, person centred, strength-based approach and work with them to achieve outcomes by mitigating barriers, and providing opportunities tailored to support our clients goals.
We specialise in complex case coordination and each Support Coordinator has their area of expertise. Clients are matched to a Support Coordinator best suited to meet their clients' goals.

Rebecca Dare

Rebecca – Specialises in Adults and Adolescents - With a Degree in Psychology and 5 years in working in the housing and homelessness sector with disadvantaged youth across DCP, worked as an ABA therapist. Bec’s experience and skillset means she work predominantly with complex forensic psychosocial clients. I work with Aboriginal and CALD community and have extensive knowledge in both NDIA and mainstream services my particular area of expertise lies in:

Release planning from incarceration (connected with social workers across all SA Correctional Facilities, Statewide NDIS Offender Services, Justice Planning Teams within NDIA)

Jason - With a strong background in Disability Services and over four years of hands-on experience in the sector, I bring a client-focused, trauma-informed approach to my work as a Level 2 Support Coordinator. I hold a Diploma in Disability and Services from Flinders University, along with multiple qualifications in community support, fitness, and safety, and have built my career supporting individuals with complex physical, mental, and psychosocial needs.
I have worked with clients with Autism, Huntington’s disease, acquired brain injury, challenging behaviours, dementia, epilepsy, and dual diagnosis mental health conditions. My frontline support work experience has equipped me with deep insight into the day-to-day realities of people living with disability. I now use this experience in my Support Coordinator role to ensure that clients can access services that are not only clinically appropriate but also genuinely person-centred and culturally responsive.
I specialise in building capacity, implementing NDIS plans, liaising with multidisciplinary teams, and navigating complex service systems to ensure participants
receive the support they need across the areas of daily living, allied health, therapy, and community engagement. I work closely with participants and their families to identify goals, resolve barriers, manage crisis situations, and link in with both mainstream and NDIS-funded services.
I am fluent in English, Mandarin, and Hokkien, and professionally proficient in Cantonese, allowing me to support clients from culturally and linguistically diverse backgrounds with sensitivity and effectiveness.
· NDIS Plan Implementation & Coordination: Experienced in managing Level 2 coordination for participants with high needs, ensuring timely connection to supports and effective plan utilisation.
· Multilingual & Culturally Responsive Support: Able to communicate fluently across five languages and work sensitively with clients from diverse backgrounds.
· Collaborative Teamwork: Skilled in liaising with service providers, therapists, families, medical professionals, and NDIA representatives to achieve holistic outcomes.
· Therapy Coordination: Coordinating updated assessments including FCA, OT, Speech, Psychology, and Physiotherapy to build strong evidence for plan reviews.
· Crisis Management & Problem-Solving: Support participants during periods of instability, service breakdown, or hospital discharge, ensuring continuity of care.
· Administrative & Documentation Skills: Skilled in accurate case noting, service agreements, provider reports, risk assessments, and plan review preparation.
· Person-Centred, Strength-Based Practice: A genuine passion for empowering individuals to achieve their goals and improve independence and wellbeing.

Lead the Way Support Coordination takes a person-centred approach to their work, meaning that they prioritise the individual's unique needs, goals, and preferences. We work closely with each individual to develop a customised support plan that is tailored to their specific needs and aspirations.

At Lead the Way, our coordinators also adopt a strength-based approach, and draw from the individuals' abilities, knowledge and skillset to increase capacity in different life domains rather than focusing on negative attributes or areas that need increasing in terms of support.

Trauma Informed
At Lead the Way Support Coordination, we understand many individuals may have experienced significant trauma. We recognise the presentation of trauma and trauma related symptoms and acknowledge the role trauma may play in an individual's life - including the impact this can have in terms of engagement with formal and informal supports.
