If you are interested in receiving Support Coordination services, here is what you can expect from the process.

Overall, the Support Coordination process is designed to be flexible and responsive to your unique needs and preferences. Your Support Coordinator will work with you every step of the way to ensure that you are receiving the services and supports you need to live a fulfilling life.

Initial Appointment

Support Plan Development

Ongoing Monitoring and Review

Once we receive your referral, the next step in the process is to have an initial appointment. This is so your Support Coordinator can understand your needs, goals, and preferences. During this appointment, you may be asked questions about your living situation, your health and medical history, your support network, and any challenges or barriers you are facing.

Once your initial appointment is complete, the Support Coordinator will work with you to develop a support plan. This plan will outline the services and supports that are required to meet your needs and goals. It may include things like healthcare services, assistive technology, housing support, employment and education support, and more.

Referral

Support Coordination is an ongoing process, and the Support Coordinator will continue to monitor your progress towards your goals and make any necessary adjustments to the support plan as needed. This ensures that you are always receiving the best possible care and support.

The first step in the process is to submit a referral, we will contact you within 24 hours of receiving your referral and discuss your plan and how you feel we can best support you!

Understanding your NDIS plan

Understanding your NDIS plan is an important part of making the most of the supports and services available to you.
Make sure you take the time to review your plan carefully, and don't hesitate to ask questions or seek clarification if there is anything you don't understand.

Here are some key things to keep in mind when reviewing your NDIS plan:

Goals: Your NDIS plan should include a list of your goals and aspirations, as well as the supports and services that will be funded to help you achieve these goals. Make sure you understand what your goals are, and how the supports and services in your plan will help you achieve them.
 

Funding: Your NDIS plan will also include information about the funding that has been allocated to you for each support and service. Make sure you understand how much funding you have available, and how this funding can be used to access the supports and services you need.
 

Review dates: Your NDIS plan will have review dates, which are the dates when your plan will be reassessed to ensure that it is still meeting your needs. Make sure you know when your plan is due for review and make a note to start preparing for your review well in advance.
 

Providers: Your NDIS plan will include a list of approved providers who can deliver the supports and services in your plan. Make sure you understand which providers are available to you, and how to access their services.
 

Flexibility: Remember that your NDIS plan is flexible and can be adjusted if your needs change over time. If you find that your plan is not meeting your needs, or if you need additional supports or services, don't hesitate to speak with your Support Coordinator about making changes to your plan.
